It could be another clone, they haven’t actually said the game is set 10 years later, if it is Snake he could have aged because of the Fox die set in... that's why he has that injection.

They did a demonstration with MGS4 on stage by pausing the footage, zooming in and rotating around the Snake model as well as changing various FX and what not to show the level of detail, and to prove the footage is all real-time generated by the PS3 hardware, keep in mind these is on ALPHA Kits, when is all said and done it will look better then this.
