why is there a question mark after break?
Because he depends on her advice for every move he makes. (In this case, every bowel movement he makes.)

this and him sitting in the classroom on 9-11 shows how much of a figurehead he really is

It looks like the 1st part reads "I think I need a bathrom break?": with a question mark after bathroom break...asking for permission possibly?

What does the second part, on the bottom of the note say??, can anybody make it out?

I wasn't just guessing, it does say "I think I may need a bathroom break? Is this possible?"

To be fair, and as Snopes points out, he's wise to ask Condoleeza Rice, someone with more experience of U.N. meetings, whether it was possible for him to just get up and go to the bathroom. You can't stand up in the middle of a meeting and say "Back in a minute Kofi, gotta go drain the lizard".
